Preston County Schools -- Technology Use Consent Form
The purpose of technology in Preston County Schools is to support learning and enhance instruction. Through computer technology people can communicate with many computers and users all over the world. It is the general policy of Preston County Schools that all computers are to be used in a responsible, efficient, ethical, and legal manner. The goal is to provide users with opportunities for unique learning experiences and excellent resources. Student users and their parents
must understand that neither PCS nor any staff member can control the content of information on the Internet. Some of
the information is controversial and may be offensive; therefore, these guidelines are provided to ensure appropriate use.
This form permits the computer user named above to use Preston County School's technology resources until graduation from PHS as long as the user adheres to the following acceptable use guidelines:
* I will use computers and other technology only for educational or job-related purposes approved by PCS.
* I will use appropriate language and polite responses.
* I understand I can access e-mail accounts at school if they relate to class assignments (e-mail may be monitored).
* I understand I cannot use any type of chat room, listserv, newsgroup, etc., unless it is a specific class assignment.
* I will not transmit or send unethical, illegal, immoral, inappropriate, obscene, harassing materials or information of
any type or use abusive language.
* I will not share my home address or phone number with another user for any purpose.
* I understand that information received on-line, unless specified, is private property; therefore, I will not plagiarize
information received in any form.
* I will not download or install any commercial software, shareware, freeware, or any other files onto any computer
unless I am authorized by the building network administrator to do so.
* I will use only files I have created or am authorized to use; therefore, I will not change, copy, rename, delete, view, or otherwise access files unless I have permission from the creator or network administrator.
* I will use only my authorized account, and I will not share my password with anyone.
* I will not interfere or disrupt other users, service, or equipment.
* I will respect network security and not attempt to bypass it.
* I will not use telecommunications provided by Preston County Schools for illegal purposes, for buying or selling goods for
personal gain, or for political purposes.
The computer user is personally responsible for his/her actions in accessing and utilizing the school's computer resources.
Any violator of the guidelines will be referred to the school's Technology Team for discipline; based upon the severity of the violation, inappropriate use of the computer can result in suspension of privileges for a
period of time or permanently. In the case of vandalism or malicious destruction of data or equipment, parents and/or the user will be expected to pay for repair and/or replacement of damaged items.

The Preston County School system may, as in any other public media systems, publish news type information and pictures such as group shots, distance shots, action shots and/or pictures from the side or back of students along with student's first names. However, the Preston County School system will not take easily identifiable head and shoulders shots of your child, and use their full names without the parent's consent. You may grant consent for the latter type of pictures and names to appear on the school's website by answering the two following questions with a yes.
1. Preston County School has my permission as parent/guardian to include my son/daughter's head and shoulder type of pictures on their website as they deem appropriate. Yes_____ No_______
